4)  $4000 fine (unpaid DEFAULT) ASAP BUILDERS INC for failure to monitor movement in adjacent buildings during excavation.  5/31/16

5)  $4000 fine (unpaid DEFAULT) ASAP BUILDERS INC for failure to provide guardrails around foundation excavation 5/31/16
